Retainer types and costs in detail

Essix (clear vacuum-formed) retainer

How it works

A clear plastic tray moulded precisely to your teeth — identical in appearance to an Invisalign tray. Worn nightly to hold teeth in position.

Included post-treatment

Usually included at end of fixed brace or aligner treatment — typically 1 upper and 1 lower

Replacement cost

£50–£120 per arch (replacement); £100–£220 for both arches

Lifespan

1–3 years with regular wear before plastic fatigues or discolours

The most commonly provided retainer type in the UK. Inexpensive to replace but must be replaced every 1–3 years. If worn only at night, most patients replace every 2–3 years. Should be stored in a rigid case to prevent damage.

Hawley retainer

How it works

An acrylic plate fitting the roof of the mouth (upper) or floor of the mouth (lower), with wire clasps around teeth and a labial bow (wire) across the front teeth. Removable.

Included post-treatment

Standard NHS retainer — typically provided after NHS brace treatment

Replacement cost

£100–£200 per arch (private replacement)

Lifespan

5–10 years if cared for

More durable than Essix but bulkier and more visible. The labial wire may allow minor tooth movement to be adjusted. Becoming less common in private practice where Essix is preferred, but still standard in NHS orthodontics.

Fixed bonded retainer

How it works

A thin wire (usually twisted stainless steel or braided fibre) bonded with composite resin to the back surfaces of the 4–6 front teeth. Not removable by the patient.

Included post-treatment

Commonly fitted to lower front teeth at end of treatment; upper fixed retainers are fitted in some cases

Replacement cost

£100–£250 per arch (rebonding if debonded; full replacement if wire needs replacing)

Lifespan

5–15+ years; may last indefinitely if well-maintained

Provides passive 24/7 retention without compliance. Most vulnerable to debonding from biting hard foods. Regular dental checks should include verifying all bonding points are intact. Lower front fixed retainer is the gold standard for preventing lower incisor recrowding.

Vivera retainer (Invisalign brand)

How it works

Clear retainer trays manufactured by Align Technology using the same materials as Invisalign. Available in sets of 3 per arch with a single digital impression/scan.

Included post-treatment

Included in some Invisalign treatment packages; otherwise additional cost

Replacement cost

£300–£500 for a set of 3 retainers per arch (from Invisalign providers)

Lifespan

6–12 months per individual tray

Higher quality material than generic Essix retainers. Sold in sets of 3, providing spares. More expensive than generic Essix but may last longer. Only available through Invisalign-certified providers.

NHS retainer costs

Retainers provided at the end of a completed NHS orthodontic treatment course are included in the Band 3 charge (£306.80). The NHS provides one set of retainers following treatment completion.

First replacement retainer after NHS treatment: NHS Band 1 charge (£26.80 in 2026) — provided your treatment was completed on the NHS and you are still within the NHS system.

If you are no longer registered with an NHS provider or have moved practices, replacement retainers will typically be charged at private rates (£50–£200 per retainer depending on type and provider).

Adults who were not treated on the NHS will pay private rates for all retainers.

If you lose or break a retainer, act quickly. The longer you go without a retainer, the more teeth may shift — and the more a new retainer may not fit correctly, requiring an impression and remake.

Long-term retainer costs: what to budget

The BOS recommends wearing retainers every night for life. For a patient completing orthodontic treatment at 20, this means 50+ years of retainer wear. Planning for replacement costs is important:

ScenarioEstimated 10-year cost
Fixed retainer only (upper + lower) — 1 replacement per arch per 10 years£200–£600
Removable Essix retainers — replaced every 2–3 years£400–£1,000 per arch
Fixed retainer + Essix backup (recommended dual approach)£600–£1,500 total

The cost of a lifetime of retainers is modest compared to the cost of retreatment if relapse occurs. A full private orthodontic retreatment costs £1,500–£6,500+. Consistent retainer wear and timely replacement is by far the most cost-effective long-term strategy.
