What is IOTN?
IOTN (the Index of Orthodontic Treatment Need) is a scoring system orthodontists use to describe how “medically necessary” treatment might be. It’s less about whether teeth look straight, and more about whether the bite or tooth position could cause problems — like teeth being at higher risk of damage, difficulty biting or chewing, uneven wear, gum issues caused by severe crowding, or certain bite problems.
In simple terms: a lower IOTN need usually means the issue is mostly cosmetic, while a higher IOTN need suggests the teeth and bite may affect health or function. That’s why IOTN is often referenced in NHS pathways and treatment priority. You don’t need to memorise the numbers — the key idea is that there’s a threshold where cases become more “health need” than “appearance”.
