Clear Aligner Compliance — The 20–22 Hour Rule
Clear aligners only work when they are in your mouth. The 20–22 hour daily wear requirement is the minimum needed to generate the sustained force that moves teeth. Miss enough wear time and your teeth won't track — treatment stalls.
Why Wear Time Matters
Each aligner applies pressure to the periodontal ligament (PDL) — the tissue connecting teeth to the jawbone. That pressure must be sustained long enough each day to trigger bone remodelling, the biological process that allows teeth to move. Research indicates a minimum of approximately 20 hours per day is required; below around 18 hours, force delivery is insufficient for the planned tooth movement.
What Counts as Wear Time
| Situation | Keep Aligners In? |
|---|---|
| Sleeping | Yes |
| Drinking plain cold water | Yes |
| Tea, coffee, juice, or alcohol | No — remove to prevent staining and damage |
| Eating any food | No — always remove before eating |
| Brushing and flossing | No — remove to clean teeth and aligners |
| Talking or singing | Yes |
| Swimming | Yes, if comfortable |
| Contact sports | No — use a mouthguard instead |
| Playing wind instruments | Clinical decision — discuss with your orthodontist |
What Happens With Poor Compliance
Consistently wearing aligners for fewer hours than prescribed leads to a predictable chain of problems:
Teeth do not move as planned — actual position diverges from the digital treatment plan.
Aligners no longer fit correctly — this is called tracking failure.
Treatment is paused while refinements (additional aligners) are made.
Total treatment time increases, often significantly.
Compliance indicators — small blue dots on some aligner brands — fade with wear time, allowing your orthodontist to monitor whether aligners are being worn as prescribed.
Practical Tips for Staying Consistent
Always carry your aligner case so you can remove aligners hygienically at meals without leaving them on a table or in a napkin.
Set a phone reminder to replace aligners within 30 minutes of finishing a meal.
Track your daily wear hours using a simple timer app or aligner compliance app.
Consolidate meals — three or four structured mealtimes beats constant snacking for keeping removal time within limits.
Brush your teeth and replace aligners immediately after eating rather than waiting.
At social events, set a specific alarm to remind yourself to re-insert aligners.
Insert aligners before bed even if you forgot to put them back in earlier in the evening.
If You Have Fallen Behind
Do not skip to the next aligner to try to catch up. Instead, extend your current tray by 2–3 days until it seats fully and comfortably. If the aligner no longer seats correctly at all — it pops off or leaves visible gaps — contact your orthodontist. Never force yourself to the next tray if the current one does not fit properly.
