Visibility and aesthetics

Lingual braces are bonded to the back surface of the teeth — the tongue-facing side — making them nearly invisible from the front in most situations. They are the most discreet fixed brace option available. Ceramic braces are bonded to the front surface of the teeth but use tooth-coloured ceramic brackets rather than metal, making them less visible than traditional braces. For patients who require a fixed appliance and want the most hidden option possible, lingual braces are typically the better choice — though they are not suitable for every case and depend on clinician training and availability.

Comfort, speech, and adjustment period

Lingual braces sit on the tongue-facing surface of the teeth and can initially cause tongue discomfort, speech changes (particularly with sounds like 's' and 't'), and some difficulty eating. Most patients adapt within two to four weeks, but the adjustment period is longer than for front-facing appliances. Ceramic braces on the front surface of teeth feel more similar to metal braces — there may be some initial sensitivity, but the tongue is not affected and speech typically adjusts quickly. Patients for whom clear and immediate speech is important (such as those in frequent public speaking roles) should factor in this adjustment period.

Cleaning requirements and cost

Both lingual and ceramic braces require thorough daily cleaning around brackets and wires. Lingual braces can make this more challenging because the brackets are behind the teeth and less easily visible when brushing. Ceramic braces allow more straightforward front-of-mouth cleaning but the brackets can stain from certain foods and drinks. Lingual braces are typically more expensive than ceramic braces because of the additional clinical skill and custom manufacturing required. Not all orthodontic practices offer lingual brace treatment — availability depends on your location and the specific clinicians in your area.
